mystery pains

Sorry for the blunt title but I've been seeing doctors for 18 months now and my problems seem a total mystery to them...

I'm a 27 year old male. Just over two years ago I started to experience some discomfort in my groin,especially while sitting, I couldn't pin it down but it felt like maybe my testicals, I now recognise the sensations as sounding very much like how prostate pain is described. I stopped masturbating for a time and mostly recovered. Around the same time I noticed my Penis had begun pointing to the left. It seems like the whole organ moved, not a bend or kink. I have occasionally experienced painful erections since then.

Six months or so later the initial problems returned and I can honestly say that in the last 18 months not a single day has gone by without at least minor pain. Last november the original symptoms were supplemented by an additional problem. I often feel an intense itching and discomfort around my lower abdomen in the front. I take this to be bladder related but I can't compare this to anything. It's quite clearly something deep that I'm feeling and not a surface/skin irretation.

I've had multiple urine and semen cultures that have found nothing. Ultrasound of bladder, kidneys, prostate and testicals were also unhelpfull. I've had some blood tests that also didn't reveal anything odd.

There's no specific physical trauma to recall. I've had no sexual partners in the year before this problem emerged and none since this time (I don't feel very functional now anyway). One thing that does concern me is that, although I masturbate maybe only twice a day, left to my own devices these sessions will last for an hour or more. I can't find anything on the physical impact of that. I do wonder about injuries that can occur through repetetive physical actions; which must be a risk as with any other part of the body; or by repeatedly sustaining an errection for that long.

Could it be a vascular problem? or some physical trauma?

Please help with any suggestions, my urologists have been entirely unhelpful and I'm feeling pretty desperate.

I should also point out that urinary function is good and has not been noticeably effected.
